*1. Acknowledgment of Risks* I, the undersigned, understand that participation in physical exercise carries inherent risks. These may include, but are not limited to, injury, illness, or other medical conditions. I acknowledge that I am aware of these risks and accept full responsibility for my child’s participation in gym activities.

*2. Medical Clearance* I confirm that my child is in good health and has received a medical evaluation regarding their ability to participate in physical exercise. I will ensure that my child will refrain from using any gym facilities if they are not feeling well or if medical conditions arise that may affect their ability to exercise safely.

*3. Supervision* I understand that, while gym staff are available to provide guidance and assistance, the primary responsibility for my child's safety lies with me and my child. I acknowledge that my child will be adequately supervised by a parent or guardian while using the gym facilities.

*4. Liability Waiver* In consideration of my child's participation in gym activities, I agree to release and discharge Swift Fitness York, its owners, officers, employees, and agents from any and all claims, liabilities, or damages arising from my child's participation in activities at the gym, to the fullest extent permitted by law.
